Why Black Women Are Underrepresented in Surrogacy — And What We're Doing About It
April 10, 2026 · 10 min read · On Fertility Ground Surrogacy
Black women are underrepresented in surrogacy — as surrogates, intended parents, and agency founders. This post addresses why and what we are doing about it.
The Information Gap
The compensation available to gestational surrogates at OFG — $60,000 to $100,000+ in base pay alone — has been widely known in some communities and almost entirely unknown in others. Black women were not excluded. They were simply never invited.
The Medical Mistrust Barrier
From the Tuskegee study to the current Black maternal mortality crisis (3-4x the rate of white women), the American medical system has not earned the trust of Black women. OFG addresses this directly with BIPOC-affirming OB referrals, doula support, and mandatory post-birth check-ins.
What We Are Doing
- Active surrogate recruitment in Black communities
- Content specifically written for Black women's concerns
- BIPOC-affirming provider referrals for every service
- Surrogates set their own compensation rate
- Post-birth check-ins recognizing Black women's postpartum vulnerability
- A Black woman at the top of the organization
From Our Founder
Bree — Founder & CEO, On Fertility Ground
This is the article I have been writing, in some form, since before I started OFG. Because this question — why are Black women underrepresented in surrogacy — is the question underneath everything I do.
Financial opportunity, medical coverage, legal protection, community, support — all of that has been sitting in an industry that was not designed to reach us. I found that unacceptable. I still find it unacceptable.
We are not finished with this work. We are doing it. And I am grateful for every woman who trusts us with her journey because of it.
